Nouvita's Care Pathway

At Nouvita Healthcare, our care pathway is designed to provide clarity, consistency, and confidence for commissioners, local authorities, and families. From the moment a referral is made, we ensure patients receive the right care in the right setting – with safety, dignity, and recovery the foundation of what we do.

Our Services along the Care Pathway

  • Nouvita Hospital in-patient care

    In-patient Services

Psychiatric Intensive Care Units (PICU) – for women detained under the Mental Health Act with a severe mental disorder in acute crisis requiring intensive therapeutic support through person-centred active engagement and treatment, involving the patient in decision-making and considering their unique goals and life circumstances.

Acute Mental Health Care – our single-gender acute wards for older adults, usually over 55 years old, provide short-term, hospital care to stabilise their mental health in a safe environment.

Our PICU and Acute wards are at our Baldock Manor Hospital in Hertfordshire.

  • Nouvita Nursing Care

    Nursing Care

24/7, nurse-led support for, typically for older adults with mental health needs, provided in safe, therapeutic settings. Our teams deliver personalised care, rehabilitation, and ongoing support to help individuals stabilise, recover, and live with dignity.

Our Nursing and Residential Care services can be a natural step-down environment for recovering patients as they begin their transition to independent living.

Nursing care is provided at our Eltisley Manor care home in St Neots, and Howe Dell Manor care home in Hatfield.

  • Nouvita therapy

    Residential Care

Safe, supportive living environments designed for longer-term stability, recovery and quality of life in an independence-focused setting.

Structured programmes help patients build independence, confidence, and skills, working toward transition back into the community.

Specialist residential care for people with autism and mild to moderate learning disabilities.

Our Residential Care services are provided at a range of sites across Hertfordshire and Bedfordshire, including The Coach House, Winnett Cottage, Windermere House, Greenwood Cottage and Lavender Lodge where admissions either come from other residential homes or family homes where individuals can no longer be supported, or from prison, hospital, or other mental health services. Care is always taken to ensure optimal patient placements with risk mitigation to other residents.

  • Nouvita Supported Living

    Supported Living

Flexible, person-centred assistance to help people live independently while receiving the right level of care with an emphasis on progression to independent living.

Service Users would have usually progressed from a residential environment where they have achieved a good standard of independence.

Our supported Living service is Windsor House in Stevenage.

  • Nouvita Independent Living

    Independent Living

Service Users have reached the end of their care pathway and are ready to live independent lives within the community, supported by community mental health services.

Our entire in-house pathway provides consistent care and familiarity throughout any journey

Why Our Care Pathway Matters

  • Consistent & Transparent: Our care pathway provides a structured process that makes decision-making easier for commissioners and less traumatic for patients.
  • Flexible & Person-Centred: Care is tailored to individual needs across their recovery journey from acute crisis support to long-term rehabilitation.
  • Recovery-Focused: Every stage is designed to help patients move forward in their journey of recovery and independence, with smoother transitions as they step down through our services.

Step-by-Step Referral and Care Process

Referrals can be made quickly and easily via our referral page. We accept referrals from NHS commissioners, local authorities, GPs, and other health and social care professionals – both local and out of area.

Within 24 hours of receiving a referral, our clinical team reviews the case. We work with referrers to gather relevant medical, psychological, and social history, ensuring we match each patient to the most suitable service.

On admission, patients receive a comprehensive assessment. This informs a personalised care plan developed with input from clinicians, the patient, and (where appropriate) their family or carers.

Patients benefit from structured therapy, clinical care, and everyday support depending on their placement (PICU, acute, nursing, residential or supported living). Care is delivered by experienced teams.

We provide commissioners with regular updates and reviews to monitor progress, adjust treatment, and plan next steps in the care pathway.

Our goal is to support safe, timely transitions – whether that’s stepping down into a less intensive setting, returning to community care, or moving into supported living. Discharge planning starts early to avoid unnecessary delays and support the patient as much as possible.
